What the 2024 Presidential Election Results Mean for Naturopathic Medicine

Dear AANP Community,

As we emerge from a significant electoral cycle, I want to update you on the AANP's ongoing work to advance naturopathic medicine and ensure our profession's voice is heard at all levels of government.

The association remains steadfastly focused on our core mission: advancing the profession of naturopathic medicine and ensuring NDs can practice to their full scope. While transitions in Washington often bring uncertainty, they also present opportunities to introduce decision-makers to the vital role naturopathic physicians play in America's healthcare system.

Our government relations team is actively engaging with the incoming administration's transition process. We are particularly encouraged by signals that prevention, environmental health, and root-cause approaches to chronic disease may receive increased attention.

While it is still unclear who will get appointments to which agencies or positions, we are aware that Robert F. Kennedy's campaign has activated a call for nominations through his Make America Healthy Again platform. While insiders are not suggesting this is how appointments will actually be filled, we will still work to make sure naturopathic physicians are represented in the process. And as appointments are made across health-related agencies and people settle into their roles, we will continue to work to ensure naturopathic perspectives are represented in these important conversations.

We are positioning ourselves to be ready as new appointments are made in both agencies and advisory committees. Areas where naturopathic expertise could particularly benefit public policy include:

- Preventive medicine and wellness approaches
- Environmental medicine and reducing toxic exposures
- Nutrition and food system policy
- Natural medicines and supplement regulation
- Integrative approaches to chronic disease

To bolster our advocacy efforts, we are building a database of NDs with distinguished expertise in these areas. We are seeking individuals with impressive credentials and a substantial body of work that would be suitable for high-level appointments. Ideal candidates will have a track record that includes published research, previous appointments to government committees, service on boards of local or national organizations, connections to the different political campaigns, or other notable accomplishments in their field. If you would be interested in potentially serving on federal committees or advisory boards (recognizing that this may involve rigorous application processes), please contact me at laura.farr@naturopathic.org.

As various agencies undergo leadership transitions in the coming months, the AANP will continue advancing our key priorities:
- Expanding state licensing for naturopathic physicians
- Achieving federal recognition of the profession, including in programs such as Medicare, Medicaid, and the VA
- Protecting access to natural medicines and therapeutic options
- Supporting research into naturopathic approaches

While some paths forward may shift with new agency leadership, our fundamental mission remains unchanged, and we are hopeful that new opportunities will arise to advance naturopathic medicine as an essential component of American healthcare.

We look forward to working with all stakeholders who share our goal of a healthcare system that emphasizes prevention, addresses root causes, and supports patients' health through natural approaches.

If you have expertise to offer, have any particular connection to Kennedy’s team, or wish to get more involved generally, please reach out to me at laura.farr@naturopathic.org.

Best regards,

Laura Farr
Executive Director
American Association of Naturopathic Physicians

Outdated Federal Laws are Creating Barriers for Naturopathic Doctors

The AANP has launched the Federal Recognition Campaign to advocate for Congress to change the outdated and limited definition of "physician" in the Social Security Act to include licensed naturopathic doctors (NDs/NMDs) so that they can be a better asset to our healthcare system.

By doing so, licensed naturopathic doctors (NDs/NMDs) will be included in the laws that govern Medicare, allowing them to help alleviate the provider shortage, fill vacant healthcare jobs, provide whole-person care to Medicare populations and underserved communities, and more.

How Federal Recognition Benefits Our Communities

Patients. Licensed NDs would be able to provide whole-person care to more patients, including:

Medicare patients - who are currently forced to either pay out of pocket or lose access to their licensed ND once enrolled in Medicare, disrupting their continuity of care

Patients in underserved and rural communities - which have very high percentages of Medicare-eligible populations

Patients in Federally Qualified Health Centers (FQHCs) or rural clinics - which have very high percentages of Medicare-eligible populations

Jobs. Licensed NDs would be able to address the primary care provider shortage and fill vacant healthcare jobs, such as those in:

Federally Qualified Health Centers (FQHCs) and rural clinics. Currently, it is economically unfeasible for FQHCs (who are tied to Medicare funding) to hire licensed NDs (who lack Medicare coverage) for unfilled positions.

Healthcare roles that require providers to complete a residency before hiring. Currently, because licensed NDs are ineligible Medicare providers, they are unable to apply to Medicare-funded residencies. This prevents them from filling more healthcare jobs that require residencies.

State Advocacy. Help provide credibility and leverage in conversations to make naturopathic medicine more accessible to more patients across the nation, including those for:

State licensure

Scope expansion

Insurance coverage

Including NDs in the VA's Whole Health program so that veterans can get access to naturopathic care

Support the campaign to get federal recognition of licensed NDs by taking action today.

LEARN MORE & TAKE ACTION HERE.

VIEW THE CAMPAIGN FACT SHEET HERE.

We're Working to Make Sure You Have Access To Homeopathy

Latest News: FDA Guidance on Homeopathy

What's happening?
Late last year, the FDA finalized its Guidance on Homeopathy, which describes how the FDA intends to prioritize enforcement and regulatory actions for homeopathic drug products marketed in the United States.

Here are some quick facts to bring you up to speed:

- The guidance is aimed at manufacturers, not physicians. It focuses on the manufacturing, distribution and marketing of homeopathic products and does not focus on the use or dispensing of them by doctors.

- We have concerns about the language. Our concern is that the language in the guidance is very vague, so it is unclear if the FDA will enforce it fairly and properly. Additionally, the FDA stated - for the first time - that homeopathic products were "unapproved new drugs". The Agency stated that it is using the designation of "unapproved new drug" as an enforcement tool to give it the authority to remove homeopathic medicines whenever necessary.

-If enforced unfairly, it will have an impact on naturopathic doctors and their patients. For example, if the FDA unfairly targets a manufacturer and the homeopathic product is then removed off the market, that product will not be available for you to utilize it in your practice.

-We are working together with homeopathic experts and organizations to protect homeopathy. We are stronger when we work together. That’s why we work with our coalition partner, Americans for Homeopathy Choice, to share action alerts alerts and strategy. It’s also why we organized a ND-specific roundtable with experts from the Naturopathic Medicine Institute (NMI), the Homeopathic Academy of Naturopathic Physicians (HANP), and the New England School of Homeopathy (NESH) to develop a unified strategy.

How can you help protect homeopathy?

Take action now: Our coalition partner, Americans for Homeopathy Choice, have launched a call to action to ask Congress to support language in the upcoming Appropriations bill that would prevent the FDA from unfairly using the “unapproved new drug” designation to arbitrarily withdraw safe homeopathic medicines. Their goal is to get 50K comments by March 24th!

COMING SOON: A practice patterns survey. The organizations in the ND-specific roundtable (mentioned above) are working together to protect access to homeopathic products. Before our work can begin, we first need to understand how and what products, methods, and remedies naturopathic doctors utilize the most. The survey will be launched in the next few weeks so please keep an eye out.

The AANP's goal is to protect the substances and methods that NDs use the most and making sure that those channels remain available to our doctors and their patients. Thank you for your support.

Curious about other AANP advocacy initiatives? Learn more here.

Pharmacy Evaluation Team Says Yes to Compounded Glutathione, Despite FDA

On June 8, 2022, the Pharmacy Compounding Advisory Committee (PCAC) – the committee that advises FDA on matters related to drug compounding – voted yes to allow for compounded glutathione to be included on an approved list for compounded prescriptions. In a surprising vote, the PCAC went against FDA’s recommendations to ban the substance for compounding purposes.

History shows that typically the PCAC follows FDA recommendations. In this case, the advisory committee did not agree with FDA’s recommendations to ban the ingredient from use for compounding. According to a news account about the meeting from the American Association of Naturopathic Physicians (AANP), ‘”One member of the PCAC committee went on to explain their “yes” vote, noting that they “found the [FDA’s] review of the efficacy [of compounded glutathione] superficial and narrow, and I found their conclusion’s [on the research available] dismissive of the positive data because the outcome wasn’t something they [FDA] were interested in looking at.”‘

Given that the PCAC usually supports the FDA recommendations, the AANP and all the stakeholders who sent in comments were more than pleased with the outcome. The AANP worked diligently to form a strong working coalition to submit relevant research and testimonies on the importance of glutathione in the naturopathic medical community. “This is a major victory for patients and providers in a process that has been stacked against natural ingredients,!” AANP wrote in a response statement following the vote. “The statements and comments from providers and patients highlighted to the committee the long, safe, and essential use of glutathione and caused several PCAC members to question the exclusively negative data brought forward by FDA staff.”

“The Institute for Natural Medicine is proud of AANP’s work to educate decision makers about the importance of compounded ingredients like glutathione,” said Michelle Simon, PhD, ND, president and CEO of INM. “The work that AANP is doing is so important to patient health. While compounded glutathione has always been an important ingredient for naturopathic doctors to prescribe, it is even more important now. Recent research shows that a majority of people who have had COVID-19, likely have a glutathione deficiency (more on that here).”

Glutathione is only one of 300 ingredients that FDA has threatened to make illegal for compounding. Compounding medications are vital to naturopathic doctors because naturopathic medicine is based on a personalized approach. They prescribe customized medications to meet the specific needs of each patient. For instance, some patients have allergies to common fillers, they may need a customized dosage or delivery system, or they simply may not be able to swallow a pill. Each of these issues present complications for the patient and interfere with compliance. Compounded medications provide a customized solution in a medical world that is all too often a mass-produced one-size-fits-all pharmaceutical medications.

To learn more about the work that AANP does to convince the FDA and voting advisory committees, like the PCAC, that limiting access to compounded substances would be harmful to patient health, please follow this link. The Institute for Natural Medicine is proud of the work that AANP does to educate physicians, the public, congressional offices, and FDA on the safety and efficacy of compounded medications and why they are important to health.

_____
This article is provided by the Institute for Natural Medicine, a non-profit 501(c)(3) organization, partnered with the American Association of Naturopathic Physicians.
FDA Seeks to Ban Compounded Glutathione, AANP Responds with Call-to-Action

Access to compounded glutathione is under review by the U.S. Food and Drug Administration (FDA) and the committee that advises the agency on this matter will meet on June 8 to discuss whether to continue to allow glutathione to be compounded.

The Pharmacy Compounding Advisory Committee (PCAC) will be reviewing only the compounded forms of glutathione, not just the ingredient as a dietary supplement, according to an email alert sent out by the American Association of Naturopathic Physicians (AANP) on Monday May 23. According to the AANP, PCAC will listen to testimonies, review comments, and consider the FDA staff’s recommendations before voting on whether glutathione can or cannot be compounded.

In event materials for the June 8 meeting, the FDA recommended weighing against glutathione being placed on the 503A Bulks List, which includes bulk drug substances that can be used in compounding. The PCAC will take this into account in their discussion.

“A ban on compounded glutathione would be disastrous," said Laura Farr, executive director of the American Association of Naturopathic Physicians, in an email to Integrative Practitioner. "FDA's recommendations are a gross intrusion on the practice of medicine and a provider's clinical expertise to tailor compounded glutathione specifically for a patient's needs and will put the health of millions of patients at risk."

According to Farr, the PCAC committee usually votes to support FDA staff recommendations. As a result, the AANP is currently working in a coalition to help prepare testimony, submit research, and mobilize comments to defend compounded glutathione. The organization is urging comments from practitioners who prescribe compounded glutathione and their patients who are impacted.

Compounded glutathione is used by patients who don't respond well to FDA approved drugs, when prescribed by providers in individualized treatment plans for dozens of diseases and conditions, ranging from Parkinson's to lung disease, and mitigating adverse effects of chemotherapy in cancer treatments (among many others), Farr said.

According to the PCAC meeting announcement, glutathione was reviewed for the following uses: skin lightening, cystic fibrosis, asthma, chronic obstructive pulmonary disease, chronic lung disease, oxidative stress, reduction of the side effects of chemotherapy, inhibition of chemical induced carcinogenesis, prevention of radiation injury, treatment of heavy metal poisoning (cadmium and mercury), acetaminophen toxicity, autism spectrum disorder, Alzheimer’s disease, Parkinson’s disease, major depressive disorder, schizophrenia, helicobacter pylori infection, human immunodeficiency virus infection, tuberculosis, otitis media, peripheral obstructive arterial disease, anemia, diabetes, and septic shock.

Practitioners can provide feedback and submit comments regarding their clinical experiences and the impact of this compounded ingredient on patient care through the AANP’s website or through the FDA website by June 7, 2022.

Read the full article at Integrative Practitioner.

Posted on May 23, 2022, 8 p.m.

The FDA is reconvening its Pharmacy Compounding Advisory Board Committee (PCAC).

On the agenda for this meeting is compounded glutathione. The agenda can be seen on the hyperlink below. Practitioners and patients can comment only until the end of May 24th, 2022.

The American Association of Naturopathic Physicians (AANP) has created an action alert link making it easy to comment.

Practitioners and patients should share how important compounded glutathione is to their practice and to their personal health.

Here is the AANP action alert that is out regarding compounded glutathione:

The FDA/PCAC request for comment docket on compounded glutathione is open until tomorrow, May 24th, for comment.

According to Healthline, it is a powerful antioxidant produced in cells. It’s comprised largely of three amino acids: glutamine, glycine, and cysteine. Levels within the body may be reduced by a number of factors, including poor nutrition, environmental toxins, and stress. Its levels also decline with age. Boosting glutathione levels may provide a number of health benefits including but not limited to helping to reduce oxidative stress, improve psoriasis, improve insulin resistance in older people, reducing cell damage in alcoholic and nonalcoholic fatty liver disease, increasing mobility for those with peripheral artery disease, reduces symptoms of Parkinson’s disease, helps with autoimmune disease, may help reduce oxidative damage in children with autism, may help reduce the impact of uncontrolled diabetes and it may also help reduce respiratory disease symptoms, among other uses.

https://naturopathic.org/page/LegislativeActionCenter#/118

https://www.regulations.gov/document/FDA-2021-N-0357-2586

https://www.healthline.com/health/glutathione-benefits

View the World Heath Net article here.

Protecting access to compounded bioidentical hormones (cBHT) is one of the many federal advocacy initiatives that the AANP works on to support the naturopathic community. The coalition we work with on this issue recently released news that the FDA likely illegally influenced the process and results of a recent NASEM study, which recommended restrictions on compounded hormone therapy.

An independent, non-partisan, third-party review by the Berkeley Research Group (BRG), commissioned by the Pharmacy Compounding Foundation demonstrates that FDA inappropriately influenced the composition, the research considered, and recommendations of the NASEM report.

Protecting access to compounded bioidentical hormones (cBHT) is one of the many federal advocacy initiatives that the AANP works on to support the naturopathic community. The coalition we work with on this issue recently released news that the FDA likely illegally influenced the process and results of a recent NASEM study on cBHT.

Read more about the what attorneys for a coalition of APC-member compounding pharmacies discovered as a result of a Freedom of Information Act (FOIA) request of FDA and NASEM.

For two decades, buprenorphine has been used as a safe, effective and life-saving medication-assisted treatment (MAT) for individuals suffering from a substance use disorder. However, medical professionals need a special DEA waiver to prescribe buprenorphine to treat substance use disorder. In many states, licensed NDs can prescribe controlled substances but cannot apply for the DEA waiver. The AANP continues to be a supporter of Mainstreaming Addiction Treatment (MAT) Act.

Learn more about the bill here.

U.S. Representative Glenn Grothman (R-WI) recently introduced a resolution (HR 116) which recognizes the benefits of vitamin D intake for the prevention of respiratory infection and COVID-19 complications, and, if enacted, would instruct the Centers for Disease Control and Prevention (CDC) and the Food and Drug Administration (FDA) to “issue guidance on vitamin D intake for the elderly and immunocompromised individuals during the coronavirus pandemic.”

The AANP commends Congressman Grothman (R-WI) for introducing a resolution before Congress that raises public awareness of vitamin D and calls for the FDA and other government agencies to study and recognize the connection between vitamin D deficiencies and COVID-19. As experts in natural medicine, licensed naturopathic doctors have long recognized and utilized the power of key vitamins, especially vitamin D, to help support the immune system. This resolution underscores the importance of safe, affordable, and natural approaches to improve health and immune resiliency. The AANP enthusiastically joins the call for our government’s top public health agencies to issue guidance on how vitamin D can help limit the negative effects of COVID-19.

To read the official press release please click here.

The AANP continues to fight for the protection of and access to natural medicine. In doing so, we’ve written a letter to the Biden administration that is specific to naturopathic medicine. This letter highlights the priorities of our profession, including how naturopathic physicians can address healthcare gaps by being included in programs such as Medicare, be utilized for patient-centered care in future public health emergencies, and lower healthcare costs.

Read our letter here.

What’s happening?

The FDA is proposing new rules - called the “Draft Guidance” - that, if adopted, could lead to the removal of many popular homeopathic medicines from the market.

This could be a serious threat to the use of homeopathy
as a healthy, alternative medicine. Our friends at Americans for Homeopathy Choice submitted a petition to the FDA urging them to
reconsider this classification. This petition is supported by 20 leading homeopathy organizations.

How can you help?

We’re submitting comments to the FDA in support of the petition. When you leave a comment through the AFHC website,
it goes directly to the FDA, as well as members of Congress and other lawmakers. Our goal is 100,000 comments.

Action Needed:

1. Click the link below to submit a comment. Read the pre-filled comment in the action alert
and modify any portion of the message that does not apply to you.

(i.e. changing "I'm writing to you because I depend on homeopathic medicines to keep me and my family members healthy" to something like "I am a licensed naturopathic physician
